Enhancing Care Transitions

The Division of Hospital Medicine has identified the discharge process as a key priority area for our quality improvement efforts in the 2009-2010 calendar years. Under the leadership of Neil Gupta, MD and Maria Novelero, MA, MPA, the division is embarking on a complete overhaul of the discharge process, with focused attention on patients on the medicine service at both the Parnassus and Mt. Zion campuses.

The Division of Hospital Medicine has been selected as one of 24 hospitals across the country to participate in the Society of Hospital Medicine's ‘BOOST’ (Better Outcomes for Older adults through Safe Transitions) collaborative. Through our involvement in BOOST, the division will be working to accomplish the following goals:

1. Identify patients at high risk for readmission or complications after discharge by using a BOOST-validated screening tool
2. Improve communication between the inpatient team and primary care provider
3. Improve patient education (including knowledge of diagnoses, medications, and follow-up care plan) at the time of discharge
4. Develop a method for enhanced communication between patients and health care providers immediately after discharge
